Newark (EWR)-Kauai (LIH) from 09/02/08-09/09/08 for $272 including all taxes.
Houston (IAH)-Honolulu(HNL) from 01/20/09-01/27/09 for $287 including all taxes.
From Newark you should easily be able to get over 10K elite qualifying miles with this fare. You generally need to earn a minimum of 25K elite qualifying miles in a calender year to earn elite status.
